Fee-for-Service
Whether it’s for school, extracurricular activities, or summer camps, Pravan Kids makes obtaining health certificates quick and convenient.
How It Works:
- Submit Your Request
Upload the required certification form through our mobile app. - Quick Turnaround
Our pediatricians will complete and certify the form within 3 business days (*certain conditions apply). - Wellness Evaluations (if needed)
- If it’s been more than three months since your child’s last physical evaluation, we’ll schedule a wellness appointment to ensure an accurate assessment.
- We can coordinate the necessary appointments for certifications requiring specialized screenings with outside providers (e.g., hearing, vision, or cardiology). Simply share the results with us through the app for final certification.
Fees and Delivery:
Health certificates are fee-for-service, charged to the payment method on the patient’s account.
Once processed, you’ll receive a digital copy via our mobile app and can pick up the printed version at our clinic.
